About the Annual Health and Medical Record

What is the Annual Health and Medical Record?
It’s the health form Scouting America requires from everyone who takes part in Scouting. It gives leaders and medical staff a participant’s health history, medications, allergies, insurance, and emergency contacts. It’s often called the AHMR, and its form number is 680-001.
Which parts do I need?
Everyone completes Parts A and B for every Scouting activity, including day camps, local tours, and weekend campouts shorter than 72 hours. Resident, tour, and trek camps, and any event longer than 72 hours, also need Part C, a pre-participation physical. Choose the 72+ hour form above to get all three parts.
Who fills it out and signs it?
Every participant needs one, youth and adults alike. Part A is signed by the participant, or by a parent or legal guardian for youth under 18. Part C is completed by a licensed physician (MD or DO), nurse practitioner, or physician assistant.
How often do I need a new one?
Update the form at least once a year.
Where does the finished form go?
Print the downloaded form, sign it, and give it to your unit leader. If you need Part C, take the printed form to your medical provider for the physical first.
Is this the official form?
Yes. ScoutKeep downloads the current official PDF directly from Scouting America and fills in your answers. If the official form changes and ScoutKeep can’t fill it in reliably, you get links to the blank forms instead. ScoutKeep is an independent product and isn’t affiliated with or endorsed by Scouting America.
Is my health information private?
Yes. Your answers never leave your browser. Nothing you type is sent to ScoutKeep’s servers or stored anywhere, and the filled form downloads straight to your device. Because nothing is saved, closing or refreshing the page clears your answers, so download the form before you leave.
