Health forms

Fill in the official health form in your browser and track what’s turned in.

This guide is for parents and guardians. It explains the Health Forms page: where to get the official Scouting America health forms, how to fill one out in your browser, and how to let your pack know your forms are turned in.

Open Health Forms from the menu.

Your forms stay private

Scouting America doesn't allow unit leaders to email or store completed health forms electronically. ScoutKeep follows that rule:

  • ScoutKeep never stores your health forms or the medical information on them.
  • If you fill out a form online, your answers stay in your browser and are never sent to ScoutKeep.
  • You turn in signed paper copies. Don't email, scan, or upload completed medical forms.

Choose the right form

The page lists two official forms under Download the forms:

  • Annual health forms: the health and medical information needed for regular activities.
  • 72+ hour camping health forms: additional forms required for longer campouts and extended adventures.

Select a form to open the official blank copy from Scouting America. Ask your leaders if you're not sure which ones your family needs.

Fill out a form online

Instead of writing everything by hand, you can answer the questions in ScoutKeep and get the official form back already filled in.

  1. Under Fill it out online, select Fill out the form.
  2. Under Which form do you need?, choose the annual form or the 72+ hour camping form.
  3. Under Who is this form for?, choose a Scout or parent from your family, or Someone else. ScoutKeep fills in what it already knows: the person's name and phone number, your pack's unit number, council, and Cubmaster's name and phone, your first two emergency contacts, and, for a Scout, your family's parents as adults allowed to pick them up. Check those answers before you print.
  4. Work through the sections: participant details, unit, insurance, emergency contacts, pickup and permissions (for youth only), health history, allergies and medications, and immunizations.
  5. In Health history, you can select Answer "No" to the rest after answering the questions that apply.
  6. Under Print and turn in, select Open the filled form to open it in a new tab, or Save as a file to download it.
  7. Print the form, then sign and date it by hand. Part C of the extended form is also completed by hand.

Nothing is saved while you work. If you close or refresh the page, your answers are gone, so download the form before you leave. If you switch to another person, ScoutKeep fills in only the answers that are still blank.

If an answer is missing or doesn't fit on the official form, ScoutKeep tells you which ones to fix before it fills in the form.

Tip: Keep your emergency contacts up to date in My Family. The online form uses your first two as the emergency and alternate contacts, skipping the person the form is for.

If ScoutKeep can't load the official form, it shows a link to download the blank form so you can fill it out by hand.

Turn in your forms

Give the signed paper copies to your Cubmaster or the leader your pack has chosen to collect them.

Mark your forms as turned in

Some packs use ScoutKeep to track which families have turned in their forms. If yours does, the top of the Health Forms page shows your family's status for the current school year: Turned in or Not turned in.

  1. Hand your paper forms to your leader.
  2. Select Mark as turned in.

This records only that your forms were provided. No medical information or files are stored. If you marked it by mistake, select Mark as not turned in. You'll also find the same status in the Health forms section of My Family, and it's the same as the health forms step in the setup checklist on your family home, so checking either one updates both.

If your pack doesn't track health forms, you won't see the status or the button. The page still has the forms and the online filler.

Keep your pack on the same page.

Start with your pack’s details and choose the tools that make sense for you.
